Must-Watch Sessions for Medicine Residents

1.KIDNEY TRANSPLANTATION & GRAFT REJECTION
Kidney transplantation brings together immunology, pharmacology and clinical medicine, making it an important topic for postgraduate examinations.
But instead of memorising different types of rejection and immunosuppressive drugs separately, it helps to understand why the immune system attacks the transplanted kidney in the first place.
Start With the 3 Signals of T-Cell Activation
T-cell activation can be remembered through three essential signals:
Signal 1 — Recognition:
The T-cell receptor recognises the antigen presented through MHC.
Signal 2 — Co-stimulation:
A second co-stimulatory signal is required for complete T-cell activation.
Signal 3 — Cytokines:
Cytokines promote T-cell proliferation and differentiation.
This three-signal model forms the foundation for understanding T-cell-mediated graft rejection.
Direct vs Indirect Antigen Presentation
Another important concept is how the recipient’s immune system recognises donor antigens.
Direct presentation: Recipient T cells recognise donor MHC directly on donor antigen-presenting cells.
Indirect presentation: Recipient antigen-presenting cells process donor antigens and present them to recipient T cells.
Understanding this distinction makes the mechanisms of rejection much easier to remember.
What About Immunosuppression?
Immunosuppression is used to prevent graft rejection while balancing the risks associated with excessive immune suppression.
The session discusses:
- Calcineurin inhibitors
- mTOR inhibitors
- Antimetabolites
- Induction therapy
- Maintenance therapy
Know the Types of Graft Rejection
Hyperacute rejection: Rapid rejection associated with pre-existing antibodies.
Acute rejection: May involve T-cell-mediated or antibody-mediated mechanisms.
Chronic rejection: Progressive graft damage occurring over a longer period.
The key is to understand the mechanism, timing and clinical significance rather than simply memorising the names.
